The Sickle Cell Community Consortium (SCCC) is developing a survey that will capture patient perspectives of patient centered outcomes.  The findings of this survey will be used to inform researchers, physicians, policy makers, funding agencies, community based organizations and other entities about the needs of sickle cell patients using the patients’ VOICE.  The research question is as follows:  Given the current need for patient centered outcomes, What are the priorities (funding, research, practice, etc.) as expressed by individuals living with sickle cell disease?

With this information, we can work together to improve the lives of individuals living with SCD. You were asked to take this survey because your opinions are valuable. Participation in this survey is purely voluntary. If you choose to participate, you do not have to answer every question. Your responses will be kept confidential and none of the information you give can be connected to you. Your identity will never be identified in any report of participant answers. Answering the survey questions will not affect your health care in any way.
