Wickenburg Community Hospital has proudly served your community for nearly 100 years, providing quality health and wellness services as a nonprofit, community-owned critical access hospital. As the hospital looks to the future, your help is needed to build a healthier future for the community.

 

Wickenburg Community Hospital is partnering with the Center for Public Health Practice and Research (CPHPR), part of the Targeted Technical Assistance for Rural Hospitals Program, to conduct a Community Health Needs Assessment. This Center is within the Jiann-Ping Hsu College of Public Health, Georgia Southern University. This assessment will help us better understand and plan for the healthcare needs of your community. We value your opinion and invite you to complete this survey. The survey should take about 15-20 minutes to complete.


Your voice matters. By taking a few minutes to complete this survey, you’ll play a vital role in shaping the future of healthcare services in Wickenburg and surrounding areas.

 

Your participation in this survey is voluntary. You must be 18 years old or older to participate. You will not receive any compensation. Please note that the hospital will not have access to your actual responses. Your privacy will be protected. All data will held in a safe file by the lead investigator at Georgia Southern University in its de-identified form. All responses will be reported in aggregate. No individual information will be reported. Participation in this study will not affect your relationship with the hospital. We also do not expect you to suffer any discomfort or risk due to participating in this study. However, if you do, you may stop participating at any time.
